About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at The Chislehurst Partnership. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the delivery of our healthcare services.

Key Responsibilities
  • Work collaboratively with our multi-disciplinary team to provide patient-centered care.
  • Conduct thorough clinical assessments, diagnose, and develop treatment plans for patients with undifferentiated diagnoses.
  • Provide expert care, including assessment and diagnostic skills, for patients within our general practice.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
  • Communicate effectively with patients, carers, and other healthcare professionals to ensure seamless care delivery.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Registered with the NMC and a postgraduate qualification at Level 7 or above relevant to your area of enhanced practice.
  • Experience working autonomously in an advanced nursing role, with a strong understanding of clinical governance and evidence-based practice.
  • Excellent written and verbal communication skills, with the ability to formulate and implement policies, procedures, and protocols.
  • Ability to work both independently and as part of a team, with a strong focus on patient-centered care.
