About the Role

We are seeking a dedicated Senior Care and Support Professional to join our team at BAE Healthcare Ltd in Southwark, UK. As a compassionate and experienced care assistant, you will provide essential support to patients in various settings, primarily through home-to-home care.

Main Responsibilities
  • Assist patients with daily living activities, including personal hygiene, dressing, and mobility, while promoting independence and dignity.
  • Develop and implement individualized care plans to meet the unique needs of each patient, working closely with healthcare professionals to ensure continuity of care.
  • Maintain accurate records of patient care, reporting any changes in condition or concerns to the healthcare team in a timely manner.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and delivery.
  • Support patients with basic IT tasks related to patient documentation and care management systems, promoting digital inclusion and empowerment.
  • Prioritize ongoing training and professional development to enhance skills and knowledge in care delivery, adhering to regulatory requirements and best practices.
Requirements
  • A valid driving license and access to a car are essential for this role, as regular travel between patient locations is required.
  • Excellent communication skills in English, both verbal and written, are necessary for effective collaboration with patients, families, and healthcare teams.
  • Previous experience in home care or working within a care home setting is highly valued, although not mandatory.
  • The ability to develop and follow care plans tailored to individual patient needs is critical, demonstrating a commitment to person-centered care.
  • A compassionate approach with a strong desire to help others is essential for success in this rewarding role.
