About You

To excel in this role, you should be a qualified Nurse with at least 2 years of post-qualification clinical experience in Adult Nursing, holding a current RN or RNLD registration. A solid understanding of national standards and regulations is essential, along with competence in technology and ability to work remotely.

The Role

">

You will play a pivotal role in community engagement, developing comprehensive care plans and working closely with service users, families, and carers. The position involves devising and implementing care plans, risk assessments, and associated documentation, while ensuring seamless coordination and integration of care services.

Key Responsibilities

  • Establish a strong presence within the community through physical visibility and accessibility, fostering open communication and trust.
  • Develop and regularly update care plans, utilizing a person-centred approach.
  • Support and supervise Community Care Leads and Healthcare Workers, ensuring adherence to clinical competencies.
  • Maintain clinical training and competencies through ongoing support, monitoring delegated clinical tasks, and being accountable for service safety and competency.
  • Participate in out-of-hours support and on-call responsibilities, ensuring availability for urgent matters.
