We are a county-wide Early Intervention in Psychosis team that provides care to people experiencing their First Episode of Psychosis. Our aim is to work in line with the principles of Early Intervention to deliver specialist care that reflects the principles of recovery and social inclusion.

Job Responsibilities

">

As a Care Coordinator Specialist, you will be responsible for Care Coordinating a caseload of approximately 15 patients who are presenting with their first-episode of psychosis or an at-risk mental state. You will provide specialist assessment, create collaborative care plans, and implement evidence-based interventions.

">

You will work alongside the patient and their families to provide them with hope for the future, enable them to develop a sense of control over their illness, and support them to re-engage with their interests. Within this role, there will be opportunities to engage in continuing professional development, service and health promotion, running groups, and working with the team to develop our service.

Person Specification

">

You will need to be a Registered Mental Health Practitioner - Mental Health Nurse/Occupational Therapist/Social Worker with essential skills including ability to travel, flexibility with working hours, and ability to attend mandatory training events.
