Our team works intensively over a period of up to three years with clients, their families, and the wider community to provide the best possible outcomes for people experiencing psychosis and to mitigate the long-term effects of serious mental illness.
Role OverviewThis is an exciting opportunity to join our dynamic and expanding Early Intervention in Psychosis team as a Care Coordinator. You will play a vital role in providing specialist assessment, care planning, and evidence-based interventions to individuals experiencing their first episode of psychosis or an at-risk mental state.
As a key member of the team, you will work closely with patients, their families, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient. You will also have the opportunity to engage in continuing professional development, service and health promotion, running groups, and working with the team to develop our service.
Key Responsibilities- Care coordinating a caseload of approximately 15 patients who are presenting with their first-episode of psychosis or an at-risk mental state
- Providing specialist assessment, creating collaborative care plans, and implementing evidence-based interventions
- Managing risk in the least restrictive manner
- Working alongside the patient and their families to provide them with hope for the future, enable them to develop a sense of control over their illness, and support them to re-engage with their interests
- Registered Mental Health Practitioner (Mental Health Nurse/Occupational Therapist/Social Worker)
- Trained in CBT approaches and/or willingness to undertake training
- Trained in Family Work and/or willingness to undertake training
- FLAP/Mentor Trained and/or willingness to undertake training
