We are seeking a highly skilled and dedicated Registered Nurse to join our Wolds and Louth Community Nursing Team. As a community staff nurse, you will be responsible for delivering high-quality care to patients in their own homes, care homes, and clinic settings.

The successful candidate will have the ability to work autonomously and as part of a team, assessing care needs, planning, delivering, and evaluating nursing care using evidence-based practice. You will also be required to work closely with the multidisciplinary team to ensure effective coordination of patient care.

Main Responsibilities
  • Deliver high-quality care to patients in their own homes, care homes, and clinic settings
  • Assess care needs, plan, deliver, and evaluate nursing care using evidence-based practice
  • Work closely with the multidisciplinary team to ensure effective coordination of patient care
  • Develop and maintain therapeutic relationships with patients and their families
  • Contribute to the development and implementation of care plans
Requirements
  • Current 1st level UK NMC adult nurse registration
  • Evidence of continuous professional development
  • Demonstration of mentorship, supervisor, or assessor qualification and its use in practice
  • Evidence of completed leadership modules/qualification
  • Evidence of V100 or V150 Community Practitioner prescribing course undertaken
Working Arrangements

The service works through the hours of 8:00 am to 6:30 pm, 7 days a week, and is covered by variable shift patterns of early shifts (8:00 am - 4:00 pm) and late shifts (10:30 am - 6:30 pm). You will be required to travel to patients' homes and clinic settings, and having access to transport is essential.

We offer a robust preceptorship and clinical skills programme with an experienced clinical education team, providing peer support and the requisite training required to fulfill the role to its true potential.

Person Specification
  • Ability to demonstrate current 1st level UK NMC adult nurse registration
  • Evidence of continuous professional development
