Job Title: Apprentice District Nurse

We are seeking a highly skilled and motivated Registered Nurse to join our Wolds and Louth Community Nursing Team. As a community staff nurse, you will be responsible for delivering care to patients in their own homes, care homes, and clinic settings.

Main Responsibilities:
  • Assessing care needs and planning, delivering, and evaluating nursing care using evidence-based practice.
  • Working autonomously and as part of a team to provide high-quality patient care.
  • Developing and maintaining therapeutic relationships with patients and their families.
  • Collaborating with the multidisciplinary team to ensure effective care coordination.
Requirements:
  • Current 1st-level UK NMC adult nurse registration.
  • Evidence of continuous professional development.
  • Demonstration of mentorship, supervisor, or assessor qualification and its use in practice.
Desirable Qualifications:
  • Evidence of completed leadership modules/qualification.
  • Evidence of V100 or V150 Community Practitioner prescribing course undertaken.
