We are seeking a highly skilled and dedicated Community Staff Nurse to join our Frailty SDEC team at Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients who require short-term medical interventions and rehabilitation.
Key Responsibilities
- Provide exceptional nursing care to patients, ensuring their safety, dignity, and well-being at all times.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to enhance patient outcomes.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and transitions.
- Participate in quality improvement initiatives, audits, and research studies to advance the field of community nursing and improve patient care.
- Maintain accurate and comprehensive patient records, adhering to Trust policies and procedures.
- Stay up-to-date with current clinical and professional issues, best practices, and research findings to provide the highest level of care.
Requirements
- NMC registration and a valid PIN.
- Experience in community nursing, preferably in a frailty or rehabilitation setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
- Basic computer skills and proficiency in electronic patient records.
