Job Summary

The Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust is seeking a highly skilled and experienced Band 6 Nurse to join our Community React Team. As a key member of our team, you will be responsible for providing high-quality, person-centered care to patients in their own homes, working collaboratively with other healthcare professionals to ensure seamless care delivery.

Main Responsibilities
  • Provide intensive packages of care to meet the needs of patients in their own homes, acting as a clinical role model to ensure high standards are embedded in clinical practice.
  • Support the clinical nursing developments within the Hospital at Home service, including IV therapies, new care pathways, and single point of access.
  • Coordinate the nursing team in the absence of the Team Leader, prioritizing workload and ensuring adequate cover.
  • Receive referrals on a daily basis, taking appropriate action to ensure patient needs are met in a safe and timely manner.
  • Communicate effectively with patients, families, and carers, respecting their privacy, dignity, culture, and human rights.

Requirements
  • Registered Nurse with extensive proven post-registration experience.
  • Evidence of complex clinical assessment and experience of working autonomously.
  • Effective communication skills, ability to prioritize, and time management skills.
  • Basic keyboard skills, phlebotomy, and making clinical decisions.
