Job Summary

We are seeking a highly skilled and dedicated Community Staff Nurse to join our Frailty SDEC team. This is a fixed-term position covering 12 months' maternity leave, working full-time hours with a flexible shift pattern to ensure 24/7/365 coverage.

About the Role

As a Community Staff Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes or in our community settings. You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate care plans that meet the individual needs of our patients.

Key Responsibilities
  • Provide expert nursing care to patients in their own homes or in our community settings.
  • Work collaboratively with the multidisciplinary team to develop and implement care plans that meet the individual needs of our patients.
  • Conduct thorough assessments of patients' needs and develop care plans that are tailored to their specific requirements.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in the development and implementation of policies and procedures that promote high-quality care and patient safety.
Person Specification

We are looking for a candidate who possesses the following skills and qualifications:

  • NMC registration.
  • Experience in the relevant speciality, with a strong understanding of frailty care and community nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to work.
  • Basic computer skills and ability to maintain accurate records.
