Key Responsibilities:
- Deliver daily team huddles and formulation meetings to ensure effective care coordination
- Conduct clinic appointments, home visits, and Mental Health Act assessments
- Work closely with the crisis team to provide comprehensive care
- Participate in SPA sessions to ensure seamless care transitions
Requirements:
- Full-time, 40 hours per week
- Monday to Friday working days
- Ongoing long-term contract
