**Job Title:** Expert Healthcare Practitioner

**Estimated Salary:** £65,000 - £90,000 per annum.

**Location:** Whittle Surgery in Chorley, Lancashire, UK.

We are seeking an experienced and skilled Expert Healthcare Practitioner to join our multidisciplinary team at Whittle Surgery. As a key member of the team, you will be responsible for delivering high-quality patient care, working autonomously, and making critical decisions to satisfy the expectations and demands of the job.

The ideal candidate will have a Master's degree (MSc) and relevant experience in primary care, with a very high level of clinical skills and competency. You should also be able to demonstrate the application of evidence-based practice and meet the requirements of the role within the Network DES.

As an Expert Healthcare Practitioner, you will work closely with other healthcare professionals to provide holistic care to patients accessing primary care services. Your duties will include undertaking clinical practice at an advanced level, assessing and diagnosing patients, prescribing medication, and referring patients to other medical specialties or secondary care when necessary.

You will also be responsible for developing and managing highly specialized programs of care and care packages, providing highly specialized advice concerning care normally beyond the scope of normal working practice. Additionally, you will communicate sensitive condition-related information to patients, relatives, and carers, initiate the process of diagnosis with patients suspected of having chronic diseases, and contribute to practice targets such as QoF, Quality Contracts, and DES/LES.
