Community Health Nurse

Job Summary: We are seeking a skilled Community Health Nurse to join our team at NHS Professionals. As a Community Health Nurse, you will be responsible for providing high-quality patient care in a community setting.
Key Responsibilities:
- Assess and monitor patient conditions, arranging diagnostic tests as necessary.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and their families to promote healthy behaviors and disease management.
Requirements:
- Current registration with the Nursing and Midwifery Council (NMC).
- Minimum of 6 months paid experience in a community environment within the last 2 years.
- Car driver with a clean license.
- Awareness of personal safety and ability to work independently.
