About the Role

We are seeking a skilled and dedicated Community Health Nurse to join our team at NHS Professionals Limited. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes or community settings.

Key Responsibilities
  • Assess and monitor patient conditions, arranging diagnostic tests as necessary
  • Recognize and respond to changes in patient condition, escalating concerns to senior staff as required
  • Manage emergency situations, using sound judgment and analytical skills
  • Develop and implement individualized care plans, working collaboratively with multidisciplinary teams
  • Provide education and support to patients and their families, promoting health and well-being
Requirements
  • Current registration with the Nursing and Midwifery Council (NMC)
  • Relevant degree or diploma in nursing
  • Experience working in a community setting, with a minimum of 6 months paid experience in the last 2 years
  • Valid car driver's license and access to a vehicle
