Job Title: Community Health Nurse

Job Summary:

We are seeking a skilled and compassionate Community Health Nurse to join our team at NHS Professionals. As a Community Health Nurse, you will be responsible for providing high-quality patient care in a community setting, working closely with patients, families, and healthcare teams to promote health and well-being.

Key Responsibilities:

  • Assess and monitor patient conditions, arranging diagnostic tests as needed.
  • Develop and implement individualized care plans to meet the unique needs of patients.
  • Provide education and support to patients and families on health management and disease prevention.
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Current registration with the Nursing and Midwifery Council (NMC).
  • Minimum 6 months of paid experience in a community environment within the last 2 years.
  • Valid driver's license and access to a vehicle.
  • Awareness of personal safety and ability to work independently.
