Key Responsibilities:- Develop and maintain effective relationships with service users, ensuring their wishes, feelings, and beliefs are central to decision-making.
- Facilitate conversations that matter most to individuals, promoting empowerment and informed decision-making.
- Connect people to relevant resources and services, enhancing their access to opportunities and support.
- Enhance the creative potential and self-esteem of individuals, promoting their overall well-being.
- Respond promptly to crises, providing timely and effective support until challenges are resolved.
- Maintain regular contact with service users, demonstrating a genuine interest in their progress and well-being.
