Job Title: Social Care Specialist

Cheshire East Council is seeking a skilled Social Care Specialist to join our Adult Social Care Teams. This temporary role, initially for 12 months, will provide an opportunity to work in a collaborative environment and develop new skills.

As a Social Care Specialist, you will be responsible for assessing individuals' needs and providing support to enhance their quality of life. You will work closely with the community team, healthcare professionals, and other stakeholders to deliver a comprehensive service.

The ideal candidate will have a strong understanding of adult social care, mental capacity, and safeguarding issues. They will possess excellent communication skills, be self-motivated, and able to work autonomously.

Key responsibilities include:

  • Conducting thorough assessments to identify individuals' needs and develop personalized support plans
  • Providing emotional support and guidance to individuals and their families
  • Collaborating with healthcare professionals and other agencies to deliver a holistic service
  • Managing complex case loads and prioritizing tasks effectively
  • Staying up-to-date with policies, procedures, and best practices in adult social care

The successful candidate will be a good team player, able to work under pressure, and possess a flexible approach to their work. Hybrid working arrangements and a car will be essential for this role.
