Role Overview

About the Role

As a key member of our team, you will be responsible for supporting adults with complex needs, providing person-centered care, and working collaboratively with multi-disciplinary teams. You will need to have excellent communication skills, decision-making abilities, and the capacity to think creatively in your work.

Key Responsibilities
  1. Supporting adults with complex needs through person-centered care and risk assessments
  2. Collaborating with multi-disciplinary teams to deliver effective support and interventions
  3. Developing and maintaining accurate and comprehensive client documentation
  4. Working autonomously within Council policies and professional practice
Requirements
  • A strong background in social care, with experience working with adults
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Good organizational and time management skills
Salary and Benefits

The salary for this role is between £36,648 and £39,186 per annum, depending on experience. Cheshire East Council offers a range of benefits, including flexible and mobile working, opportunities for learning and development, and a supportive and well-managed team environment.
