About the Role
We are seeking a dedicated and compassionate Healthcare Assistant to join our team at Modality Partnership. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and supporting the health and wellbeing of our patients.
Key Responsibilities
- Assess patient needs and provide pre-screening activities and healthcare for chronic diseases
- Support the wider nursing team to promote patient health and wellbeing
- Conduct health checks, including BP and ECG monitoring, and take blood samples
- Provide health and wellbeing advice and promotion
Requirements
- Basic IT knowledge and skills
- Understanding of person-centred care
- Recent previous experience in direct patient care
- Professional approach to work and strong team player
