About the Role

Job Summary

The successful candidate will provide support and care to service users, promoting their independence, dignity, and well-being. You will work as part of a multidisciplinary team, developing personalized plans and strategies to meet the unique needs of each individual.

Responsibilities

  • Providing personal care and support to service users, including administering medication, assisting with daily living tasks, and promoting physical activity
  • Developing and maintaining positive relationships with service users, families, and carers
  • Contributing to the development and implementation of individual care plans, ensuring they are tailored to meet the specific needs and goals of each service user
  • Collaborating with healthcare professionals, social workers, and other stakeholders to ensure seamless coordination of care and support
  • Participating in training and professional development opportunities to enhance skills and knowledge
