Physical Health Lead Specialist

We are seeking a highly skilled Registered General Nurse (RGN) to join our team as a Physical Health Lead Specialist. As part of our multidisciplinary team, you will be responsible for delivering exceptional care to our patients with physical health needs. This role offers a challenging and rewarding opportunity to make a real difference in the lives of those in our care.
About the RoleThe successful candidate will have a strong understanding of the latest nursing practices and a commitment to patient recovery. They will be responsible for assessing, planning, and implementing care plans that meet the physical health needs of our patients. The Physical Health Lead Specialist will also be responsible for monitoring and intervening in emergency situations, providing information and advice on medication regimes, and working closely with visiting GPs.
Responsibilities and Accountabilities- Develop and maintain effective relationships with patients, families, carers, and other healthcare professionals.
- Conduct thorough assessments of patients' physical health needs and develop individualized care plans.
- Implement and evaluate the effectiveness of care plans in collaboration with the multidisciplinary team.
- Provide education and support to patients and their families to promote healthy living and self-care.
The salary for this role is £40,000 - £45,000 per annum, depending on experience. Benefits include NHS and employee discount schemes, free meals on shift, and free parking.
