Overview

We offer a unique opportunity for a skilled and motivated individual to develop advanced clinical skills and competencies in palliative care. As a Community Palliative Care Specialist, you will play a vital role in supporting patients with life-limiting illnesses in their own homes, ensuring they receive high-quality care and symptom management.

Key Responsibilities

  • Assess and manage patients with complex symptoms, working closely with the District nurse teams and out-of-hours providers.
  • Develop and implement personalized care plans, focusing on patient-centered care and maximizing quality of life.
  • Collaborate with healthcare professionals, including GPs, consultants, and other specialists, to ensure seamless care coordination.
  • Provide education and support to patients, carers, and healthcare professionals, promoting best practices in palliative care.
  • Maintain accurate records and contribute to audit and research activities, ensuring high standards of care and service delivery.

Requirements

  • Degree-level education in nursing or a related field; registered with the NMC and possessing at least 3 years' post-registration experience.
  • Proven experience in palliative care, preferably within a community setting, with expertise in symptom management and patient assessment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including patients, carers, and healthcare professionals.
  • A full UK driving license and access to a car are essential for this role.
