Job Summary

This is an exciting opportunity to join Trinity Hospice as a Clinical Nurse Specialist in Palliative Care. As a key member of our community team, you will have the chance to develop advanced clinical skills and provide specialist palliative care to patients in their own homes.

Main Responsibilities

Assess patients requiring palliative care in the community and contribute to their total nursing care and management of symptoms. Participate in the development and delivery of management plans in conjunction with the primary health care team. Liaise with healthcare professionals working in the neighbourhoods and other members of the Trinity Clinical Nurse Specialist Team.

As a Clinical Nurse Specialist, you will have the opportunity to work autonomously and as part of a team to provide specialist palliative care to patients in the community.

Person Specification

We are looking for a highly motivated and experienced nurse who has a strong background in palliative care. The ideal candidate will have a degree in nursing and be registered with the NMC. They will also have experience of working in a community setting and be able to work independently and as part of a team.
