Nursing Care Coordinator: An Exciting Opportunity
We are seeking a highly motivated and compassionate individual to join our team as a Nursing Care Coordinator. This role involves working as part of our nursing team, providing personal nursing care and clinical interventions to patients in the inpatient hospital care setting.
Main Responsibilities
- Provide direct patient-centred care, adhering to Trust policies and procedures.
- Develop and maintain effective relationships with patients, families, and healthcare teams.
- Contribute to the assessment of care needs and delivery of planned care.
- Participate in training and development opportunities to enhance clinical skills and knowledge.
- Monitor and report changes in patient condition, escalating concerns as necessary.
Requirements
- NVQ Level 3 in Health & Social Care or equivalent qualification/experience.
- Care Certificate.
- English & Mathematics at GCSE grade A-C (9-4) or level 2 functional skills.
Estimated Salary
This role carries an estimated salary of £28,000 - £33,000 per annum, depending on experience.
