About the Role
We are seeking a skilled Mental Health Practitioner to join our Newcastle Adult Community Mental Health Team at the Lyme Brook Centre.
The successful candidate will be responsible for conducting assessments, triaging referrals, and providing support to service users in need.
This is an exciting opportunity to work with a multidisciplinary team, including Psychiatrists, Community Mental Health Nurses, and Social Workers, to provide high-quality care and support to our community.
Key Responsibilities
Assessing service users in need of mental health support
Triage and managing referrals to ensure timely access to care
Providing support and interventions to service users, including home visits and follow-up appointments
Liaising with other professionals to ensure seamless care and support
Person Specification
Essential
- A qualified Mental Health Nurse, Occupational Therapist, or Social Worker (minimum diploma level)
- Experience of working therapeutically in community settings
- Knowledge of various approaches to mental health assessment
