Job Description

The role of the Field Care Supervisor is to provide high-quality care support services by carrying out various functions in the field, including assessing and reviewing individual services, and supervising, instructing, and supporting care workers. Working closely with care coordinators to plan realistic and effective care worker rotas, taking into account the specific needs of individuals and the skills and attributes of care workers. Regularly reviewing individual services, including repeated and renewed needs and risk assessments. Participating in multi-disciplinary reviews of service users' needs as required. Shadowing new care workers in the field to ensure their competence. Conducting on-site observations and evaluations of care worker practice as part of routine supervision, in response to performance concerns, or as directed. Field Care Supervisors are expected to spend most of their working time in the community performing their primary functions, but their role will also involve some office-based administrative work.

Qualifications

What You Need

You would need experience in conducting initial risk and needs assessments for individual service users, either in their home or in other settings such as hospitals or residential/nursing homes. Working in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure seamless delivery of holistic packages of individual care support.
