About the Role

The Human Support Group Limited is seeking a highly skilled and experienced Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a vital role in supporting the delivery of high-quality care support services to our clients.

Key Responsibilities

As a Field Care Supervisor, your primary responsibilities will include:

  • Supporting the planning and delivery of care support services to clients in their homes and communities.
  • Working with care coordinators to develop and implement effective care plans that meet the individual needs of clients.
  • Conducting regular reviews of client services to ensure that they are meeting the required standards.
  • Providing supervision and support to care workers in the field to ensure that they are delivering high-quality care.
  • Participating in multi-disciplinary reviews of client needs and developing plans to meet those needs.

Requirements

To be successful in this role, you will need to have:

  • Experience in carrying out initial risk and needs assessments in respect of individual service users.
  • Working in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of holistic packages of individual care support.

Language

This role requires fluency in English.
