Essex Partnership University NHS Foundation Trust is seeking a highly skilled and dedicated Senior Community Nurse to join our Specialist Dementia and Frailty Team. As a key member of our team, you will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working closely with other healthcare professionals to deliver a comprehensive service.
Key Responsibilities:
- Conduct comprehensive assessments and reviews of care for patients with dementia and frailty.
- Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams.
- Provide expert advice and guidance on complex care needs, ensuring seamless transitions between services.
- Supervise and mentor junior staff, promoting a culture of excellence and professional development.
Requirements:
- Registered Mental Health Nurse (RMN) qualification.
- Experience in performance management and supervision of lower-banded staff.
- Knowledge of health legislation, principles of CPA, and risk assessment/management.
- Excellent IT skills, including Word, Excel, PowerPoint, and Outlook.
