Senior Community Children Nurse

We are seeking an experienced and enthusiastic NMC registered nurse to join our team as a Senior Community Children Nurse. This is an exciting opportunity to work within a multi-disciplinary team closely with staff from other agencies to promote and maintain high standards of care for children and adolescents with learning disabilities / developmental delay and complex needs.
The successful candidate will be responsible for assessing developmental / associated health needs, planning, implementing and evaluating appropriate programmes of care. They will also work with children, their families, carers, staff teams and other professionals in a variety of community settings with ongoing support.
Main Responsibilities:- To take responsibility for allocated referrals from source to discharged in line with agreed protocols and documentation.
- To conduct initial assessments of physical, psychological and social well-being in order to accurately identify the child's needs and associated health needs.
- To carry out assessments utilising evidence-based tools and where appropriate to provide more detailed specialist assessment of specific areas of difficulty.
