Senior Community Children Nurse

Betsi Cadwaladr University Health Board NHS is seeking an experienced and enthusiastic NMC registered nurse to join our Conwy & Denbighshire Children's Learning Disability Nursing Service.
About the RoleWe are looking for a dedicated and passionate nurse to work within a multi-disciplinary team, closely with staff from other agencies, to promote and maintain high standards of care for children and adolescents with learning disabilities / developmental delay and complex needs.
Key Responsibilities- Assess developmental / associated health needs, plan, implement and evaluate appropriate programmes of care.
- Work with children, their families, carers, staff teams and other professionals in a variety of community settings with ongoing support.
- Take responsibility for allocated referrals from source to discharged in line with agreed protocols and documentation.
- Conduct initial "choice" appointments (or equivalent) with carers to agree suitability of the service to meet the child's needs and agree outcome-focused goals.
- Undertake an initial assessment of physical, psychological and social well-being in order to accurately identify the child's needs and associated health needs.
