About the Role

We are seeking an experienced and enthusiastic NMC registered nurse to join our team in the Conwy & Denbighshire Children's Learning Disability Nursing Service.

Key Responsibilities
  • Assess developmental and associated health needs of children and adolescents with learning disabilities and complex needs.
  • Plan, implement, and evaluate appropriate programmes of care in collaboration with multi-disciplinary teams and other agencies.
  • Conduct initial choice appointments with carers to agree suitability of the service and outcome-focused goals.
  • Undertake assessments of physical, psychological, and social well-being to accurately identify children's needs and associated health needs.
Requirements
  • NMC Registered Nurse
  • Evidence of continued professional development
  • Post-registration experience
