Job Summary

We are seeking a skilled Paediatric Community Nurse Specialist to join our team. As a Band 4 member, you will work closely with a multi-disciplinary team in the Brent Children's Community Nursing Team.

This is an exciting opportunity for a qualified nursing professional to further develop their skills within the community setting.

About the Role
  • Assess and plan delegated care for patients
  • Monitor long-term conditions and well-being of service users, escalating interventions when necessary
  • Implement and monitor clinical care, tracking outcomes
  • Administer medication according to regulatory guidelines
  • Refer patients to appropriate services and community-based care support
  • Provide therapeutic relationships to meet physical, emotional, psychological, social, cultural, and spiritual needs
  • Enter data into clinical systems as per Trust guidance and policies
