Job Title: Field Care Supervisor

Are you passionate about delivering high-quality care services to individuals in their own homes? We are seeking a skilled Field Care Supervisor to join our team at City and County Healthcare Group Ltd.

Job Summary:

The Field Care Supervisor will be responsible for supporting the delivery of care services by carrying out assessments and reviews of individual care plans, and supervising care workers in the field. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Carry out assessments and reviews of individual care plans to ensure they are meeting the needs of service users
  • Supervise care workers in the field to ensure they are delivering high-quality care services
  • Work with care coordinators to plan and implement care worker rotas
  • Participate in multi-disciplinary reviews of service users' needs
  • Shadow new care workers to ensure they are competent in their roles
Requirements:
  • Experience in carrying out initial risk and needs assessments
  • Ability to work in partnership with service users, their families, carers, and healthcare professionals
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
