Job Title: Field Care Supervisor

Constance Care Limited is seeking a highly skilled and dedicated Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a vital role in supporting the delivery of high-quality care services to our clients.

Key Responsibilities:
  • Support the planning and delivery of care services to meet the needs of our clients.
  • Conduct regular reviews of individual services to ensure they are meeting the needs of our clients.
  • Supervise and support care workers in the field to ensure they are providing high-quality care.
  • Work collaboratively with care coordinators to plan realistic and effective care worker rotas.
  • Participate in multi-disciplinary reviews of service users' needs.
Requirements:
  • Experience in carrying out initial risk and needs assessments.
  • Ability to work in partnership with service users, their families, carers, advocates, and healthcare professionals.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
