Home Care Specialist

**Job Summary:**
We are recruiting a skilled paediatric nurse to join our PATCH team as a Home Care Specialist. This role offers a unique opportunity to make a real difference in the lives of children and their families, while contributing to the development of innovative models of care.
**Responsibilities:**
- Providing assessment and treatment for children at home through telephone reviews and/or home visits
- Developing and delivering educational resources to professionals working across the urgent care pathway for children
- Contributing to the review and audit of existing pathways of care for minor conditions
**Requirements:**
- Postgraduate qualification in paediatric nursing or equivalent experience
- Registration with the NMC as a qualified paediatric nurse
