We are seeking a dedicated and compassionate Care and Support Specialist to join our team at SweetTree Home Care Services.
Job SummaryThis role involves providing high-quality care and support to individuals with learning disabilities and autism in a home-based setting. As a Care and Support Specialist, you will work collaboratively with clients, their families, and our multidisciplinary teams to deliver tailored support that promotes independence, dignity, and well-being.
Key Responsibilities- Assisting with daily living tasks, such as meal preparation, personal care, and household chores.
- Supporting individuals with medication management and administration.
- Encouraging community engagement and participation through outings and activities.
- Collaborating with healthcare professionals to coordinate medical appointments and treatments.
- Maintaining accurate records and documentation to ensure seamless care delivery.
To be successful in this role, you will need:
- Minimum 3 months' experience working with individuals with learning disabilities and autism.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Flexibility to work varied shifts, including evenings and weekends.
