Advanced Care Coordinator

Job Summary:
We are seeking a highly skilled and experienced Community Nurse to join our team in providing high-quality healthcare to patients in their own homes.
The successful candidate will work as part of a multidisciplinary team, delivering care and support to patients with complex health needs. As a key member of the team, you will be responsible for assessing patients' physical, psychological, and social care needs, developing individualized care plans, and working closely with other healthcare professionals to ensure continuity of care.
About the Role:
- To provide skilled nursing care to patients in their own homes, using evidence-based practice and clinical guidelines.
- To undertake comprehensive assessments of patients' needs and develop personalized care plans to promote independence and self-care.
- To act as a named nurse for assigned patients, collaborating with the community nurse team lead and other healthcare professionals to deliver seamless care.
- To stay updated on Trust clinical and operational guidelines and policies, ensuring adherence and promoting best practices.
- To uphold the NMC Code of Conduct and other professional guidelines in all aspects of your work.
Requirements:
- Registered Nurse (RN) with current NMC registration.
- Recent clinical experience in a community setting, with a strong track record of teamwork and collaboration.
- Demonstrated knowledge of out-of-hospital care and the role of the community nurse.
- Able to drive, with a valid license.
