We are seeking a skilled Social Worker to join our Children with Disabilities Team, providing specialist support to children and young people with significant and profound disabilities.
As a Social Worker in this team, you will work closely with children, families, and other professionals to assess needs, develop care plans, and provide direct support to children and young people in their homes, schools, and communities.
You will be responsible for:
- Assessing the needs of children and young people with disabilities and developing care plans to meet their needs
- Providing direct support to children and young people with disabilities, including home visits, school visits, and community-based support
- Working closely with families, carers, and other professionals to ensure that children and young people receive the support they need
- Participating in multi-agency meetings and contributing to the development of care plans
- Providing training and support to colleagues and other professionals on disability-related issues
We are looking for a Social Worker who is passionate about working with children and young people with disabilities, has excellent communication and interpersonal skills, and is able to work effectively in a team.
