About the Role

We are seeking a skilled Mental Health Wellbeing Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to provide high-quality care and support.

Key Responsibilities:

  • Assess and manage clinical risk, providing prompt local assessment, triage, and risk management for patients presenting with mental health problems.
  • Deliver evidence-based interventions, including group work and one-to-one sessions, to support patients in achieving their recovery goals.
  • Work collaboratively with primary care, partner agencies, and wider mental health services to provide a seamless service to patients.
  • Represent the team on specific projects and working groups as required.

Person Specification

We are looking for a candidate with:

  • Knowledge of principles of assessment and risk management in mental health.
  • Experience of working in a mental health setting and delivering evidence-based interventions.
  • Relevant professional qualification in health or social care, with current registration.
