About the Role

We are seeking a skilled Mental Health Community Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to our patients.

Key Responsibilities
  • Work as a key member and resource of the MDT core team.
  • Develop relationships with Primary Care, partner agencies, and wider mental health services.
  • Design and deliver evidence-based recovery-focused mental health interventions and groups.
  • Assess and manage clinical risk, and provide prompt local assessment, triage, and risk assessment.
  • Represent the team on specific projects and working groups as required.
Requirements
  • Relevant professional qualification relating to Health or Social Care in social work, nursing, OT with current professional registration.
  • Experience of working in a Mental Health setting, assessment of people presenting with mental health problems, and assessment and management of clinical risk.
  • Ability to work flexibly as the need of the service demands.
  • Positive attitude and ability to act on feedback, ability to praise and be supportive to others, and delivery of evidence-based interventions in mental health.
