About the Role

We are seeking a highly skilled and dedicated Mental Health Community Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to our patients.

Main Responsibilities
  • Design and Deliver Evidence-Based Interventions
    • Develop and implement recovery-focused mental health interventions and groups that cater to the diverse needs of our patients.
  • Build and Maintain Relationships
    • Establish and maintain strong relationships with primary care providers, partner agencies, and other mental health services to ensure seamless care coordination.
  • Clinical Caseload Management
    • Manage a blended caseload of patients with common mental health disorders, severe mental illness, and those with emotional needs, providing evidence-based interventions and support.
  • Collaboration and Communication
    • Attend regular multidisciplinary team meetings, participate in professional development opportunities, and provide signposting to adjunct services as needed.
  • Carer Support and Guidance
    • Offer support and guidance to carers of patients accessing our services, ensuring they receive the necessary support and resources.

Person Specification
  • Essential Criteria
    • Knowledge and Skills
      • Knowledge of principles of assessment of people with mental health problems.
      • Knowledge of principles of risk assessment and management.
      • Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation.
  • Additional Criteria
    • Able to meet the travel requirements of the post.

  • Experience and Qualifications
    • Experience of working in a mental health setting.
    • Assessment of people presenting with mental health problems.
    • Assessment and management of clinical risk.
    • Working within a multi-disciplinary team.
    • Managing own caseload.
