About the Role

We are seeking a highly skilled and experienced mental health professional to join our Early Intervention in Psychosis (EIP) Service as a Care Coordinator. The successful candidate will work closely with a multidisciplinary team to provide high-quality care and support to individuals experiencing first-episode psychosis.

Key Responsibilities

  • Provide assessment, care planning, and coordination of care for individuals with first-episode psychosis
  • Work collaboratively with the multidisciplinary team to deliver a recovery-based approach to care
  • Develop and maintain effective relationships with service users, carers, and other stakeholders
  • Contribute to the development and implementation of care plans that promote independence and self-management
  • Participate in training and professional development opportunities to enhance skills and knowledge

Requirements

  • Education to degree level or equivalent in a relevant field (e.g., mental health nursing, social work, occupational therapy)
  • Registration with the relevant professional body (e.g., NMC, HCPC)
  • Post-registration experience in acute mental health services, including experience of working in crisis situations and emotionally demanding environments
  • Excellent communication and interpersonal skills, with the ability to work effectively with a diverse range of stakeholders
