Job Title

We are seeking a highly skilled Care Coordinator to join our Early Intervention in Psychosis (EIP) Service - PATH (Psychosis: Prevention, Assessment and Treatment in Hertfordshire).

Job Responsibilities

As a Care Coordinator, you will be responsible for providing assessment, care planning, and care coordination to adults experiencing first episode psychosis. You will work as part of a multi-disciplinary team to deliver a high-quality social care service, promoting recovery and choice for service users.

Main Duties

  • Provide assessment, care planning, and care coordination to adults experiencing first episode psychosis.

Requirements

To be successful in this role, you will need to have a degree in a relevant field, such as Mental Health Nursing, Social Work, or Occupational Therapy. You must also be registered with the relevant professional body (NMC/HCPC) and have post-registration experience, including solid Band 5 experience.
