Job Title: Advanced Clinical Practitioner - Frailty Specialist

Doncaster & Bassetlaw Teaching Hospitals NHS FT is seeking an experienced Advanced Clinical Practitioner to join our team as a Frailty Specialist. This is an exciting opportunity to work with our multidisciplinary team to provide comprehensive geriatric assessment and care for frail older adults.

Main Responsibilities:
  • Reduce hospital admissions by providing clinical expertise and collaborating with MDT colleagues in primary and secondary care.
  • Utilize patient pathways, including virtual wards and community services, to safely discharge patients into the community, reducing length of stay and improving patient outcomes.
  • Provide high-quality care for frail older patients within the Emergency Department, ATC, and Ward A4.
Requirements:
  • 5 years post-registration experience
  • Highly specialist knowledge underpinned by theory and experience
  • Complex reasoning, critical thinking, reflection, and analysis to inform assessments, clinical judgments, and decisions
  • Evidence of management skills gained through theory and/or practice
  • Research and development activity
  • Clinical expertise in the specific role
  • Autonomous experience of running and managing nurse-led clinics
  • Experience of leading an audit project cycle
  • Experience of teaching and mentoring
Qualifications:
  • First-level Registered Nurse, physio, or pharmacist - current registration
  • IT literate
  • Up-to-date knowledge of professional issues
  • Evidence of mandatory training updates
  • Core MSC level 7 modules, including APACS, NMP, CXR interpretation, and Managing acutely unwell/managing medical conditions
  • Leadership training
  • Teaching and assessing qualification
  • Masters-level learning and at least 2 modules appropriate to the role
Interpersonal Skills:
  • Demonstrates excellent interpersonal skills
  • Ability to delegate
  • Ability to problem-solve, analyze, compare, and interpret complex information and take appropriate action
  • Prioritizes clinical workload and assesses associated risks, taking appropriate action
  • Demonstrates good organizational skills
  • Ability to present information to individuals and groups as required using appropriate resources
  • Able to demonstrate the flexibility required of the role
  • Reliable
  • Experience of leading services
  • Development and reviews together with managing operational roles
