Healthcare Support Worker

We are seeking a talented and caring Healthcare Support Worker - Reablement to join our team at the Epsom and St Helier University Hospitals NHS Trust. This is an exciting opportunity to work in a fast-paced and rewarding environment, delivering high-quality patient care and rehabilitation, enabling patients to achieve their therapy goals and promote independence prior to discharge home.
The successful applicant will be working alongside physiotherapists, occupational therapists, nurses, pharmacists, and a GP to deliver high-quality patient care and rehabilitation. Key responsibilities will include carrying out therapy plans as per OT/PT recommendations and working as part of the MDT to ensure patients' safe and timely discharge home.
Required Skills and Qualifications
- Experience of a caring role
- Experience of working as a member of a team within health or social care
- Experience in patient documentation
