Community Nurse Case Manager Leader

We are seeking a dedicated and experienced Nurse Case Manager to join our team in the North West. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to clients with complex needs. This is a full-time position, working 37.5 hours per week, Monday to Friday, with some flexibility required. The role is home-based with travel across the region.
The ideal candidate will possess excellent clinical skills, including experience with complex care, such as peg feeds, ventilators, and tracheostomy management. A car driver is essential for this role, which requires regular travel across the North West. We offer a competitive salary of £38,000 per annum, plus the opportunity to earn up to £6,000 per year in tax-free bonus based on targets. Other benefits include 25 days annual leave plus bank holidays and an extra day for your birthday, pension, and support around CPD/revalidation.
