About the Role

We are seeking a highly skilled Hospital Social Worker to join our experienced team of practitioners in London.

This position requires a Social Work Qualification with a minimum of 2 years' post-qualified experience.

Key Responsibilities:
  • To independently undertake cases to facilitate safe discharges in a timely manner.
  • Assessing risk factors and creating care plans/packages (Care Act 2014) for patients to be discharged at the right time.
  • Duty work, attending board rounds, and working closely with health colleagues to achieve best outcomes for patients.
About You

To be suitable for this role, you will need a degree in Social Work (Degree, DipSW, CQSW) with a minimum of 2 years' experience. Experience working with older people, discharges, and community-based work is desirable.

Able to attend the office a minimum of 3 times a week is essential.
