Job Title: Pediatric Community Nurse - Complex Care Specialist

We are seeking an experienced Pediatric Community Nurse to join our team at Thornbury Community Services in Oxford. As a Pediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.

Key Responsibilities:
  • Provide individualized care and support to young people with complex needs
  • Work collaboratively with the multidisciplinary team to develop and implement care plans
  • Conduct regular assessments and reviews to ensure the best possible outcomes for our clients
  • Communicate effectively with families and carers to ensure a seamless transition of care
Requirements:
  • Minimum of 6 months experience in the last 2 years in a Pediatric Community Nursing role
  • Recent experience in suctioning, mechanical cough assist, care of gastrostomy, and change of gastrostomy
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
