About the Opportunity

We are excited to offer an exciting opportunity for a Community Support Services Coordinator to join our team at Nottingham Community Housing Association Ltd.

As a Community Support Services Coordinator, you will be responsible for coordinating and delivering community-based support services to individuals with mental health needs.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with a range of stakeholders, including service users, their families and carers, and external agencies.

Key Responsibilities

  • Coordinate and deliver community-based support services to service users
  • Develop and maintain relationships with service users, their families and carers
  • Contribute to the development of individual care plans and risk assessments
  • Participate in multi-agency meetings and planning sessions
  • Monitor and report on the well-being of service users

Requirements

  • Degree or diploma in social care or related field
  • Experience of working in a community-based setting, particularly with vulnerable adults
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and respect the dignity of service users
  • Willingness to work in a fast-paced environment and adapt to changing circumstances
