Join Our Team as a Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at Hounslow and Richmond Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care and making a real difference in the lives of our patients.

About the Role

As a Community Nurse, you will be responsible for providing skilled nursing care to housebound patients with complex needs, enabling them to remain in their own homes. You will work closely with specialist nurses, allied health, and social care professionals to meet the needs of our patients without unnecessary attendance in A&E or hospital admission.

Key Responsibilities

  • Deliver high-quality nursing care to housebound patients with complex needs
  • Work collaboratively with the Community Nursing Team to plan, implement, and evaluate individual programmes of care
  • Provide first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care
  • Work independently but as part of the Community Nursing Team covering a particular geographical area
